Position Summary
The RN Field Assessor is responsible for face to face assessing with members to evaluate the medical needs of the member to facilitate the member’s overall wellness and help them obtain the services they need to thrive. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment.
- Minimum of 10-15 assessments per week dependent on need, may be more. Assessments take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Assessments include the member’s level of work capacity and related restrictions/limitations.
- Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality.
- Consults with supervisor and others on team in overcoming barriers in meeting goals and objectives. Utilizes case management processes in compliance with regulatory and company policies and procedures.
- Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation.
- The RN Assessor will also be expected to mentor new hires on the team, once, a level of proficiency has been attained in their role.
Required Qualifications
- Must hold an active unrestricted Registered Nurse license in the state of New Jersey
- Willing and able to travel up to 75% of the time within Essex, Middlesex, Mercer, Somerset, Union, and surrounding areas. Must be comfortable completing Field based assessments for all socioeconomic status and settings
- 2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required
- Successful completion of the NJ Choice certification is a contingency of employment. (As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.”)
- Must be able to work Monday - Friday, 8:00am - 5:00pm EST
Preferred Qualifications
- Assessment and Case management and/or discharge planning experience
- Managed Care experience
- Crisis intervention experience
- NJ Choice certification
- Clinical interviewing
- Experience using personal computer, keyboard navigation, navigating multiple systems and applications; and using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.)
Education
- Minimum Associates Degree in Nursing required
- BSN preferred
Pay Range
The typical pay range for this role is:
$72,627.36 - $155,538.24
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
