Community Support/Employment Specialist - Outreach

We are looking for a Community Support/Employment Specialist to join our Assertive Community Treatment (ACT) Team. The ACT Community Support Specialist is responsible for the provision of personal and home skills training, as well as training in socializing and copy skills. Activities are focused on the recovery and rehabilitation of the ACT team member. This position is full-time, and non exempt. This position requires participation in assigned schedule evening and weekend coverage in addition to providing 24/7 on-call crisis intervention services as assigned.

WHAT YOU'LL DO:

  • Assist clients with developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services such as review incoming referrals and complete assessments as assigned and Provide direct skills training based on needs and assessments of the team member
  • Identify barriers as a result of a person's mental health issues and develop strategies to overcome those issues.
  • Conduct all phases of vocational and rehabilitative services including but not limited to: engagement, job search, job support, goal planning, safety planning, evaluating and summarizing progress achieved.
  • Assist participants with obtaining information about their benefits (SSI, Medicaid, etc.) and how they will be affected by employment and educate family members or other participant supports.
  • Assist participants with understanding an employer's job requirements and how to manage situations that cause symptoms to increase.
  • Conduct job development and search activities directed toward participants' interests and capabilities

WHAT YOU'LL NEED:

  • Bachelor's degree (BA or BS) with at least 30 semester hours or equivalent quarter hours in a human service field (sociology, social work, counseling, psychology, human services) required.
  • Minimum of one year of recent work experience with persons with severe and persistent mental illness; knowledge of major mental illness from a bio-psychosocial perspective.
  • Basic knowledge psychotropic medications and symptom management.
  • Experience in the development of teaching material and teaching clients, families, caregivers and community members.
  • Demonstrate skills in applying the principles of psychosocial rehabilitation. Experience in crisis intervention.
  • Evenings, weekends and occasional nights and early mornings are required to meet needs of clients.
  • Acceptable criminal and abuse background checks or a DHS Waiver.
  • Maintain a valid Drivers License and be approved by Vera French CMHC's insurance carrier; all Vera French employees that transport a Vera French client in his/her personal or agency vehicle must also have a D-3 Chauffeur's License. Employees are required to maintain a minimum personal vehicle coverage that equates to $100,000 each person and $300,000 each accident.
  • May require working with difficult emotional and physical management of aggressive behavior.

Additionally, you are hereby authorized to make any investigation of my personal history, educational background, military record,
motor vehicle records, criminal records, and credit history through an investigative or credit agency or bureau of your choice. I
authorize the release of this information by the appropriate agencies to the investigating service. I authorize and give permission for
the individuals listed as references to provide written information and/or verbally discuss my background with Vera French
Community Mental Health Center, or any of its affiliates.


This authorization, in original or copy form, shall be valid for this and any future reports and updates that may be required.
I understand that passing the background check is a condition of employment. A negative background check can be grounds for
dismissal, even if an offer has been made to me and I have been hired.
