The Executive Director of Clinical Care Coordination & Integration, will be responsible and accountable for the planning, development, implementation, and ongoing management of the primary care department.
The Executive Director of Clinical Care Coordination & Integration will lend leadership and administrative expertise to provide an environment that encourages the highest quality healthcare. This will require a high level of clinical integration and coordination of care between the departments of CCH.
EDUCATION & EXPERIENCE
- Bachelor’s Degree required; master’s degree in pertinent discipline preferred.
- Valid State of Michigan registered nurse licensure required.
- Minimum of five (5) years of recent experience managing a clinic/primary care setting required; hospital setting experience preferred.
- Minimum of five (5) years of leadership experience or progressive responsibility required; experience managing multi-disciplinary (clinical and non-clinical) staff preferred.
- Experience in a community mental health, public health or FQHC setting highly preferred.
- Proficient with Electronic Medical Records system; Athena experience preferred.
