Join our team as a Case Manager with our Targeted Case Management program in Winona, MN!
BASIC PURPOSE: Reporting to the Program Coordinator, the Case Manager is responsible for coordinating services for clients in extended foster care. The Case Manager assists the clients in gaining access to needed social services, mental health, rehabilitative, educational, health, vocational, recreational and related services, including but not limited to, volunteer services, advocacy, transportation and legal services.
Essential Duties & Responsibilities
• Provide case management services to include developing an individual service plan, assisting the client in obtaining needed services through coordination with other agencies and assuring continuity of care.
• Assess the delivery, appropriateness and effectiveness of services on a regular basis.
• Ensure that all documentation is completed according to Targeted Case Management policies and procedures and current governing standards. Develop, document and maintain client records such as assessments, treatment plan, progress notes and contacts.
• Attends and participates in all required staff meetings including individual supervision and engage in treatment discussions with both internal and external treatment providers in a timely manner.
Qualifications
ESSENTIAL KNOWLEDGE, SKILLS & ENVIRONMENT:
Education, Training & Work Experience: Holds a bachelor's degree in social work, psychology, sociology or a closely related field from an accredited four-year college or university in a field other than social work, psychology, sociology or a closely related field, PLUS one year of supervised experience in the delivery of social services to children as a social worker in a public or private social service agency.
Specialized Knowledge & Skills: Knowledge in mental health interventions and skills training. Skills in identifying and assessing a wide range of children’s needs. Knowledge of local child welfare and a variety of community resources and effective use of those resources for the benefit of the child. Candidate must possess knowledge of Trauma Informed Care and be able work with youth using this skill set. Candidate must have the ability to build appropriate rapport with clientele and form age appropriate limits and boundaries.

Work Environment & Physical Demands: Services will be provided at Family & Children’s Center, the client’s home or in the community as appropriate.
