Social Worker
Quaboag Rehabilitation is currently seeking a FT, Social Worker.
Position Summary: The Social Worker will provide social services to our residents, which may include: Admission process, facility adjustment, psychosocial needs, resident grievances, family grievances, MDS assessments, counseling, IDT meetings, care plans, discharge planning, facilitate discharge procedures, policies and procedures, federal and state compliance.
Primary Job Responsibilities And Duties
- Completes social history for residents assigned to caseload.
- Acts as a liaison between residents, family, and staff for any concerns.
- Assures that any personal care needs are communicated to the family with special attention to clothing needs.
- Participates as part of the inter-disciplinary team in providing interventions to resolve behavior, mood or psychosocial problems and develops and implements plan of care.
- Participates in the assessment for falls and restraints.
- Assures that residents are informed of their rights and assist residents in exercising their rights if the resident desires.
- Documents social work interventions a minimum of every three months for residents on caseload.
- Facilitates discharge planning based on the goals of the resident and family.
- Maintains a list of community resources and assist residents and family in contacting agencies as needed.
- Attends annual in-services required for all employees.
- Follows all safety, security, infection control and hazardous material policies and procedures and performs all tasks to assure resident and personal safety and protection of co-worker and facility property.
- Foster a climate, policies, and routines that enable patients/residents to maximize their individuality, independence, and dignity and the highest practical level of physical, mental, and psychosocial well-being and quality of life.
- Educate staff regarding the role of Social Services and the psychosocial needs of patients/residents and their families/significant others including the challenges of aging and disability.
- Identify and monitor community changes and opportunities such as legislation, regulations, and programs that impact nursing home patients/residents.
Qualifications
· Master’s Degree in Social Work (MSW), preferred.
· Licensure: LICSW, LCSW, or LSW licensure.
· Bachelor’s Degree in Social work or related human services field, required.
· A minimum of one year experience in long term care providing direct social services, preferred.
