CCS-Rehabilitative Support Specialist Level 1

General Statement of Duties:

The CCS-Rehabilitative Support Specialist Level 1 position serves as a vital member of the CCS team providing individual skill development in areas of home care, home maintenance, budgeting, wellness activities and enhancement services to support clients in how to reach their recovery goals.  The CCS-Rehabilitative Support Specialist Level 1 serves people at various stages of their recovery from mental health, substance use, and co-occurring needs by embracing the person's strengths to meet their identified needs.

Job Responsibilities:

  1. Be an active member in client/consumer and annual review plans.
  2. Participate in staff training, read materials, program forms, and other program documents.
  3. Engage in ongoing staff training, coaching, skill development, and performance evaluation.
  4. Ensure clients receive quality services.
  5. Provide services in the community including people's homes and other community locations.
  6. Appropriately document all contact with people being served and team members to adhere to program requirements.
  7. Responsible for accurate documentation and maintaining a case file as required by agency policy, government regulations, and applicable local, state, and federal codes.

Qualifications:

  1. Must possess experience in human services and case management, minimum bachelor's degree preferred in related field of social sciences or social work.
  2. Prior experience in leading or managing treatment teams in a human service program preferred.
  3. Valid Wisconsin driver's license and automobile with insurance required in good standing.
